
Most RPM programs don't fail because they lack data.
They fail because they misunderstand what data is for.
A common pattern: teams celebrate how many readings they collect, then act surprised when clinicians ignore it. The system becomes a dashboard museum—beautiful charts, low action.
Published evidence supports a more sober view: RPM can improve outcomes in certain contexts, but results vary depending on program design, components, adherence, and workflow integration. Meta-analyses in heart failure, for example, have found reductions in HF-related hospitalizations and sometimes mortality, but also highlight heterogeneity and the importance of how programs are built. And there are also trials where "more monitoring" didn't translate to better outcomes—showing that measurement alone doesn't guarantee impact. JAMA Network
So what actually breaks RPM?
Four usual failure points:
- Generic data without clinical context — Numbers without a protocol are just numbers. Programs need cohort-specific thresholds and "what happens next" rules.
- Alerts without prioritization — If everything is urgent, nothing is. Alert fatigue isn't a minor UX issue; it's the system teaching clinicians to stop trusting the stream.
- Data outside workflow — If remote data lives in a separate app that isn't part of daily clinical practice, it becomes ceremonial tech.
- No outcome linkage — If the program can't show fewer escalations, fewer readmissions, better adherence, improved QoL, or better staff efficiency, it won't survive beyond pilot phase.
The key shift is from raw metrics to operational insight.
Not "more data," but "the right data, shaped into decisions." This is where continuous monitoring research inside hospitals is a useful analogy: continuous systems can trigger earlier alerts than intermittent checks, but you still need sensible alert logic and response pathways to avoid noise overload. RPM that wins long-term compresses the distance between signal and action:
- individualized baselines
- trend detection (not single readings)
- clinically meaningful thresholds
- clear escalation workflows
- documentation + review accountability
Outcome-driven RPM is not a sensor problem. It's a care design problem.
